    Kaohsiung Veterans General Hospital, in collaboration with National Yang Ming Chiao Tung University and Jiu Health Mobile Technology Co., Ltd., has developed the "Integrated Person-Centered Care Communication and Collaboration Platform" (Yu Qu Tong Gong Platform). Since its introduction, it has become an essential tool for enhancing patient safety. This platform, based on the core technology of a "Universal Whole-Person Whole-Process Care Intelligent Support Mobile System," successfully addresses communication barriers and information silos in the care process through three main aspects: cross-professional collaboration, data sharing, and AI-assisted health record keeping.

    In traditional healthcare systems, communication between professionals often relies on telephone and social messaging apps, which have limitations in terms of patient privacy protection, message retention, and follow-up tracking. The introduction of the Yu Qu Tong Gong Platform establishes a new digital communication model for "elderly-centered" holistic healthcare, ensuring timely transmission and correct understanding of medical information, significantly reducing medical errors and safety risks caused by poor communication.

    The platform applies large language models and speech recognition technology to assist clinical staff in generating accurate nursing records and team meeting minutes. This not only shortens handover time but also improves record accuracy, reducing patient safety incidents caused by information transmission errors. In terms of actual effectiveness, data shows a 25% reduction in task processing time, allowing professionals to focus more on direct care, thus improving care quality and patient safety.

    The platform's cross-professional collaboration function establishes a seamless care transition system, linking hospitals, communities, and long-term care institutions to ensure continuity of care information. For example, in home-based acute care, an average of 1-2 effective communications per case were added for 25 cases of home-based hospitalized patients, significantly improving monitoring and timely intervention for high-risk patients, preventing potential safety incidents. In geriatric outpatient clinics, the platform helped medical staff quickly access complete medical histories and care information for 111 dementia patients during follow-up visits, saving an average of 8.5 minutes per consultation, reducing the possibility of diagnostic errors.

    Notably, the introduction of this platform has increased employee satisfaction, with the reduction in work stress directly reflected in improved care quality and patient safety. Additionally, the platform is estimated to save approximately NT$7.5 million annually, demonstrating its economic benefits while enhancing patient safety.

    Currently, the platform has integrated discharge preparation, long-term care, and community services, covering over 24 professions, 800 professionals, and 1,270 cases. Using medical centers as technical pivots, it has been promoted to other counties and cities, with 14 institutions and health bureaus piloting in Taitung County. This cross-regional cooperation model ensures consistent and safe care standards across different levels of medical institutions, jointly constructing a borderless patient safety network.

    As an important link in smart hospitals, the Yu Qu Tong Gong Platform not only realizes improved medical efficiency but also establishes a comprehensive patient safety assurance system through systematic communication and collaboration mechanisms. The successful application of this innovative technology fully demonstrates how AI technology can free up healthcare professionals' hands while achieving the goal of comprehensive, continuous, and safe integrated care.
